This customer service request system may not be used for issues which involve information protected by privacy considerations, which require verification of identity, or which require more information than can be provided by the system. Some examples would be starting and turning off utility accounts, Emergency Medical Service patient information, court case information, and personal information regarding specific employees, among others.
The system provides the option, but does not require that the customer provide their address, phone number, or email contact information. Some service requests can be handled without that information, and we will make every reasonable effort to do so. However, responding adequately to many service requests may require the collection of additional information, which will require that the contact information be provided.
